The Community Team Learning Disabilities (CTLD) provides a range of services to vulnerable adults over 25 whose primary need is associated with a Learning Disability. The team consists of social workers, social work assistants and welfare and well-being practitioners.
We can provide information, advice and support as well as an assessment for adults and carers so that we can provide you with the right advice and support that will meet your needs.
We work in partnership with Hywel Dda University Health Board and a range of local community and third sector organisations.

Your GP
If your need relates to your health, your doctor can discuss options and refer you to relevant services or the local Community Learning Disability Team (Hywel Dda University Health Board).
You may be signposted, to an appropriate early help and preventative service that provides support to adults and their carers across Carmarthenshire. This service will support you by connecting you with preventative services or other support in your community.

- Emotional & Practical Support
- Information and advice about how to access support for financial and housing Support
- Information about how to access Community-Based Resources

If your needs cannot be met by information, advice and support from your community you may be eligible for an assessment under the Social Services Well-Being (Wales) Act 2014 ( SSWBA)
A member of the team will speak to you about the next steps and will make arrangements to meet with you.
We’ll visit you to talk about:
- Your current situation
- What matters most to you
- What you’d like to achieve
You can have someone with you for support, like a family member or friend. If you don’t have anyone but don’t want to be alone, we can help find someone to support you.
Your assessment may be shared with other professionals so we can understand your needs and suggest the best ways to help. Sometimes, we may arrange follow-up appointments or refer you to health professionals in the Community Learning Disability Team for extra support.
If your assessment shows you have eligible needs under the Social Services Well-Being (Wales) Act 2014 ( SSWBA), a social worker will work with you to create a Care and Support Plan. This plan will lists the services you’ll receive and focuses on your goals in areas like:
- Physical and mental health
- Emotional well-being
- Personal safety
- Education, training, and recreation
- Family and relationships
- Community involvement
- Rights and entitlements
- Social and economic well-being
- Accommodation
Your plan will be reviewed regularly to make sure it still meets your needs and helps you achieve your personal outcomes.
Workers will support you to understand what matters to you by identifying your strengths and capabilities which is essential for tailoring your care and support.
Workers will involve you in decision-making, ensuring that your preferences and personal circumstances are considered.
Once a Care and Support plan is in place and services are meeting eligible needs, the plan will be reviewed, the minimum requirement is an annual review however this can be more regular depending on your circumstances.
