Residential Reablement (Ty Pili Pala)
Residential Reablement (Ty Pili Pala) Reablement is a short‑term, goal‑focused service designed to help people regain independence after illness, injury or a change in circumstances.
It is located within Llys y Bryn Care Home in Llanelli but operating separately from the main home.
A dedicated staff team works with an Intermediate Care multidisciplinary team. Who meet weekly to:
- Review your progress
- Look at what support is still needed
- Plan your safe return home
The aim is to help you regain or strengthen your skills, or learn new ways of managing daily tasks, so you can return home independently.
The service is mainly for people aged 65 and over who:
- Want to remain living in their own home
- Need a period of supported recovery after illness or injury
- Have potential to improve their independence
However, adults aged 50+ may also be considered.
The unit also helps prevent unnecessary hospital admission or early moves into long‑term care.
Support depends on your goals, health and abilities. It may include:
- Washing and dressing
- Making a drink or snack
- Taking part in social activities
- Occupational Therapy or Physiotherapy
- Equipment
- Medication support
- Advice
- Confidence‑building
Staff encourage you to do as much as possible for yourself in preparation for going home.
A GP or health professional from the Home First Team will temporarily manage your medical needs while you stay at Tŷ Pili Pala.
This may include:
- General health and wellbeing support
- Prescribing medication
- Medical assessments
When you leave, you will return to your own GP practice.
Residential reablement is short‑term, usually:
- 1–6 weeks
Your length of stay depends on your goals and progress. Some flexibility is possible, but occasionally a longer stay may be needed.
There is no charge for the Residential Reablement service, however your need for the service will be reviewed on a weekly basis.
Charges may apply if:
- You stay longer than 6 weeks after reaching your potential, if this happens you will be consulted.
- Residential reablement is no longer suitable and you need a different service such as long term care.
You would be assessed before any changes.
If you access any of the services provided such as hairdressers or provision of daily newspapers, you will need to pay the current charges.
Visitors are welcome at any time; however, you need to consider that you may be participating in rehabilitation sessions. We also ask that mealtimes are avoided by visitors for the comfort of other residents
All the bedrooms have en-suite facilities, there is a communal lounge and kitchen area where you can make hot drinks and snacks at any time.
