Home Care (Domiciliary Care)

Home care is also known as Domiciliary Care, it is support provided to people in their own homes to help them stay independent and live well. The aim is to make everyday life easier and safer, so individuals can continue living comfortably in their community.

 

Home care is available to adults aged 18 and over who are assessed as needing support. An assessment will look at your individual needs and circumstances. If you meet the eligibility criteria, we can provide help with daily tasks so that you can remain as independent as possible.

The support you receive will depend on your assessed needs. Home care services can include:

  • Help with personal care, such as washing, dressing, or getting ready for the day
  • Support with meal preparation, nutrition and hydration
  • Help with taking medication, if this is agreed as part of your care plan
  • Support to get out into the community and stay socially connected

Every person’s support package is different and is tailored to their individual needs.

You will need an assessment to access Help at Home (domiciliary care), you will need a needs assessment from the local authority’s social care team.
Referrals can be made by:

  • You (self‑referral)
  • Family members or carers
  • Health professionals
  • Other organisations or agencies

During this assessment we will visit you to talk about your day to day life and what matters to you.

The assessment looks at your personal circumstances, strengths, and support needs to help decide which services are most suitable for you.

During your assessment, other support options may be suggested such as:

 

If your assessment shows that you are eligible for support, you will be fully involved in creating a personalised care and support plan.

This plan will explain:

  • What support you will receive
  • How often carers will visit
  • What goals you want to achieve

It will also include details of who will provide your care. This may be:

  • A care provider commissioned by the council, or
  • The council’s own in‑house home care service
You also have the option to receive a Direct Payment. A Direct Payment is money provided to people who are eligible for community care services and would prefer to arrange and manage their own support.

Most people will need to pay something towards the cost of their care. This will depend on a  financial assessment which will look at your income and savings.

For people who are not eligible for support from Social Services, or who prefer to make their own arrangements, there are a number of Home Care agencies in the area which provide both personal care and practical support.

You should make sure that any agency you consider is registered with the Care Inspectorate Wales (CIW). Any care agency you employ should give you written information about the service they will provide to you and how this will be delivered. Make sure you are clear about the charging arrangements before you agree to having a service provided.
