Reablement at home

Reablement is a short-term service that helps people regain their confidence, skills, and independence after an illness, injury, or a period where daily tasks have become more difficult. Our team supports you to do as much as you can for yourself, safely and independently.

 

Reablement is for adults who normally live independently at home but now need some extra support. You may benefit if you:

  • Want to return home or continue living at home
  • Need support or equipment to help with daily tasks
  • Are likely to improve with short-term, focused support
  • Are medically stable
  • Have had an illness, accident, or time in hospital

An assessment is required to see if reablement is right for you.

Reablement staff will visit you to help with everyday tasks such as:

  • Washing and dressing
  • Moving around your home and outdoors
  • Preparing meals
  • Household tasks
  • Social activities

You may also receive personalised exercises to help improve strength and mobility. Support continues until you can manage independently or your goals are reached.

If you have been in hospital or visited the emergency department, hospital staff may refer you to a team of professionals, such as nurses, therapists and social care workers (known as the Intermediate Care Multi-Disciplinary Team (ICMDT)).

This team checks what help is needed and decides if Reablement would support you when you return home.

The assessment looks at your personal circumstances, strengths, and support needs and decide if the services is suitable for you. 

During this assessment we will talk about your day to day life and what matters to you.

  • Talk about the goals that matter to you
  • Create a plan to help you reach those goals
  • Review your progress regularly
  • Adjust your support as needed

If you are eligible, you will help create a personalised support plan. Most reablement programmes last up to 6 weeks, though many people progress sooner.

Reablement is free for up to 6 weeks.

Charges only apply if:

  • You meet your goals before 6 weeks (charges begin from the date your goals are met)
  • You no longer need reablement

Your support continues beyond 6 weeks — if so, this will be discussed with you in advance

We monitor your progress throughout the programme. If you still have eligible care and support needs at the end of the reablement period, we will arrange the ongoing care and support you require.
